EDUCATIONAL REQUIREMENTS AND QUALIFICATIONS:
- Graduate of an ACPE accredited school of pharmacy with a Bachelor of Science Degree in Pharmacy or Doctor of Pharmacy Degree is required.
- Must currently be licensed with the Indiana Board of Pharmacy as a Pharmacist.
- Must possess and maintain Basic Life Support (BLS) or obtain prior to providing direct patient care.
- Must possess the clinical skills necessary to make judgments, therapy recommendations, provide consultations to other health professionals
- Must possess the ability to read, write, compute, comprehend and implement written and verbal instructions.
- Must meet hospital annual education and employee health requirements and regularly attend department meetings to remain informed on current issues.
- Must be in good physical and mental health and possess a professional demeanor.
- Must possess the ability to learn, change and adapt as the fields of pharmacy and health care evolve and change.
- Must possess the ability to interact, cooperate with and communicate with all health professionals in a constructive and pleasant manner.
- Must possess familiarity of applicable regulatory standards as they apply to pharmacy.
- Must possess familiarity with federal and state rules, regulations and laws as they apply to pharmacy.
- Must demonstrate competency in making clinical decisions regarding principles of growth and development over the life span.
PRIMARY DUTIES:
- To interpret the drug order written by a practitioner and transmitted to the pharmacy or generated via CPOE.
- Evaluate physician drug orders for appropriateness with regard to patient condition, organ function, disease state, and concomitant therapies to promote optimal patient outcomes. Examples include, collection and interpretation of information regarding patients’ allergies, organ function, drug history, and other information pertinent to drug therapy
- Process physicians’ orders including, but not limited to, order entry into dispensing computer system, appropriate product selection, compounding, labeling and repackaging with completion of appropriate records; delegating product preparation activities to pharmacy technicians when appropriate, within legal limits and hospital policies and procedures.
- Consult with and provide information and guidance to various Medical, Nursing, and other Health Care Professionals concerning drugs and their appropriate use.
- To follow all state and federal laws, guidelines and regulations governing the practice of pharmacy and the provision of hospital services to the public.
- To check all drug orders for accuracy, safety and appropriateness of therapy and to intervene whenever the drug order is not appropriate. Computer programs as well as written information may be used to assist detection of drug allergies, drug interactions and drug incompatibilities.
- To dispense and correctly bill outpatient employee prescriptions.
- To compound oral and topical products as needed.
- To compound IV’s and chemotherapy IV’s, maintaining a sterile work area and using aseptic technique.
- To enter new drug orders on the computer and verify technician and nursing drug order entry.
- To provide pharmacokinetic monitoring and dosing for the professional staff.
- Package or supervise the packaging of all unit dose medication for inpatient and outpatient use.
