My Chart Terms and Conditions

By establishing a MyChart account, I agree to these Terms
and Conditions and any future revisions to them.  I also agree that these Terms and Conditions
make up a written agreement between Woodlawn Hospital and its affiliates,
including Woodlawn Hospital, Akron Medical Center, Argos Medical Center, Fulton
County Medical Center, Shafer Medical Center, and Woodlawn Medical Specialty
Clinic, (“Woodlawn Hospital”) and me documenting my informed consent for the
use of MyChart.

EMERGENCIES AND URGENT HEALTH CARE MATTERS

I understand that MyChart and/or physician-patient email
should NEVER be used for emergencies or urgent health care matters.  If I
believe I am experiencing an emergency or urgent or life-threatening health
condition I will either phone my physician, go directly to the Emergency
Department of a local hospital, or call 911.

USE OF MYCHART

MyChart can/may provide the following services:

  • Ability
    to request/schedule appointments, cancel appointments with 24 hours
    advance notice, view past appointments and view upcoming appointments;
  • Ability
    to view portions of my medical record such as allergies, medications,
    immunizations, medical history, health summary, current health issues and
    recent visit information;
  • Access
    to test results;
  • Electronic
    communication with physicians via secure messaging regarding health
    concerns, such as: non-urgent medical questions, prescription questions,
    test results questions, visit follow-up questions, referral requests and
    preventive care reminders;
  • Requests
    for prescription refills;
  • Pay
    medical bill(s) from MyChart account; and
  • Financial
    account information; and
  • Links
    to other sites that may provide information on health topics.

Other services may be added as Woodlawn Hospital develops
the MyChart web portal.

I understand and agree that MyChart may not be
used for:

  • Emergent appointment requests;
  • Any
    purpose at all if my last office visit with my Woodlawn Hospital provider
    was more than three years ago. These will require a telephone call,
    and possibly an office visit, to Woodlawn Hospital.

If my physician is out of the office or unavailable to
respond to my MyChart message, I understand and agree that my message may be
routed to another Woodlawn Hospital provider.  I also understand that Woodlawn
Health providers may send me messages via MyChart. I understand that I will be
able to tell when my messages have been received.  It is my responsibility
to monitor these messages and respond in an appropriate manner. I must provide Woodlawn
Hospital with a valid email address and update Woodlawn Hospital if I change my
email account. Woodlawn Hospital may send me emails via my personal email
address to notify me that I have new MyChart messages. Messages sent via my
personal email will not contain personal health information. I understand that Woodlawn
Hospital cannot be responsible for information accessed by others to whom I
have given access to my email.

I agree that all messages I send through MyChart will be
regarding my health care and conditions or the health care and conditions of a
person for whom I have proxy access. I understand that asking for advice for
any other friend or family member is not allowed.

MyChart may include access to a video visit feature. A video
visit is a real time connection to a provider in the form of a video feed,
where both parties are visible to each other. Patients between the ages of 14
and 17 should not use the video visit feature for consultation or treatment
related to mental health issues, substance use disorder, pregnancy related
care, or sexually transmitted disease. Additional terms and conditions apply.

PERSONAL IDENTIFICATION CODE AND PASSWORD

The first time I log on to MyChart I will be required to
create a unique user ID and a password. I agree to keep my activation code, if
one is provided to me, and my user ID and password confidential. Anyone with
access to my user ID and password would have access to my personal health
information and the personal health information of my proxies; they would also
be able to add to my personal health information and send messages to my
providers as if they are me. It is my responsibility to change my password if I
believe it has been compromised or shared with someone else. I understand that
it is recommended that I change my password frequently. Every time I log into
MyChart, my user ID and password will be logged in an audit trail in the
computer system.

I understand that if Woodlawn Hospital discovers that I have
inappropriately shared my access code or password or that I have abused my
access privileges, Woodlawn Hospital may terminate my participation in MyChart.
I understand that Woodlawn Hospital takes no responsibility for and disclaims
any and all liability for damages arising from a breach of medical record
confidentiality resulting from my sharing or losing my access code or password.

DISCLAIMER FOR INFORMATION PROVIDED ON LINKS TO OTHER SITES

I understand that Woodlawn Hospital may provide links or
reference to other sites, but Woodlawn Hospital has no responsibility for the
content of such other sites and shall not be liable for any damages or injury
arising from that content. Any links to other sites are provided as merely a
convenience to the users of this web portal.

AVAILABILITY AND HOLD HARMLESS

I understand that MyChart may not always be available to me
due to system maintenance, back-ups or failures. Access is provided on an
“as is available” basis and I am not guaranteed access at any
particular time.  I also understand that email is not a confidential means
of communication.  I agree to waive any rights that I may have against Woodlawn
Hospital and any supplier or operator of the MyChart technology for any loss of
information due to technical failures and/or unintended breaches of
confidentiality because of unauthorized access to my information as a result of
my decision to communicate with Woodlawn Hospital in this manner.

FEES

There is no charge for use of MyChart.

CONFIDENTIALITY

Both Federal and State laws govern the confidentiality of
medical information. Woodlawn Hospital is dedicated to keeping your medical
information confidential. Please read Woodlawn Hospital’s Notice of Privacy
Practices for more information about how Woodlawn Hospital may use and disclose

Except as provided in the Notice of Privacy Practices, Woodlawn
Hospital will obtain your consent to forward patient-identifiable information
to a third party.

SECURITY

Woodlawn Hospital has implemented the following security
features:

  • Encryption:
    We use 128-bit Secure Socket Layer to encrypt the communication from your
    web browser to our secure web server.
  • Dedicated
    web server for MyChart: We separate the web server and electronic medical
    record machines. In the unlikely event that our server is compromised, no
    health information would be exposed since it is not saved on the web
    server. Patient information is stored only in the electronic medical
    record, behind a firewall.
  • No
    Caching on the client side: We disable caching of data in the browser and
    force the web pages to expire immediately. This prevents a follow-on user
    from viewing pages that you had previously viewed while using MyChart.
  • Firewall:
    Personal health information is stored in the secure electronic medical
    record, protected from the Internet by a firewall.
  • Session
    Timeout: After a period of inactivity, the logon session will time out.
  • Monitoring:
    We constantly monitor the web server for evidence of unauthorized break-in
    attempts. We apply the latest security patches, as they become available.

TERMINATION OF MY MYCHART ACCOUNT

I understand that I may disenroll from using my MyChart
Account at any time by submitting a request through MyChart, or contacting the
MyChart helpdesk at 1-855-22-3648.  I also understand that if Woodlawn
Hospital determines in its sole discretion that I have violated this agreement
and terms and conditions of use and/or have abused the use of MyChart, Woodlawn
Hospital reserves the right to discontinue my use of MyChart.

AGREEMENT

By establishing a MyChart account and checking this box I am
agreeing to these Terms and Conditions on my behalf and on behalf of the
patient, if I am acting as their legal representative.  The use of this
“check box” is my signature on this agreement.  I understand that Woodlawn
Hospital may update these Terms and Conditions from time to time.  The
current Terms and Conditions will be posted and available on the MyChart web
site.  It is my responsibility to review them.  My continued use of
MyChart will indicate my agreement with any changes.
