MyChart Proxy and Access Resources
What is a Proxy?
Proxy access allows those such as parents, legal guardians, spouses, and those with healthcare power of attorney to log into their personal MyChart account and then connect to medical information for the patient they care for and represent. Available access is based on the age of the patient and proper authorization.
Please note that once requested, granting proxy access can take up to five (5) business days.
Full Access (Patients 0-13 Years)
Provides parents and legal guardians full access to all MyChart features through their own MyChart log in. A child of age 13 years or younger cannot have a personal MyChart Account.
To request full proxy access to a child’s MyChart information, a parent or legal guardian must review, sign, and submit the MyChart Proxy Request Form.
Limited Access (Patients 14-17 Years)
Provides parents and permanent legal guardians with limited access to MyChart through their own MyChart log in. Limited access does not require the teen’s consent. When the child turns 14, MyChart Proxy access is automatically reduced to “limited” for parents and legal guardians who already had full proxy access.
NOTE: Parents/permanent legal guardians with proxy access to an account for this age group will have limited access to certain medical information. As a result of various legal requirements and technical limitations, some access will be limited such as being unable to cancel appointments or see scheduled appointments, order medications, see existing medications, request records, or see test results.
To request limited proxy access to a teen’s MyChart information, a parent or legal guardian must review, sign, and submit the MyChart Proxy Request form.
**Teens age 14-17 account for self: With parental authorization at the age of 14, teens may create their own account. To request teen patient access, the parent/legal guardian must review, sign, and submit the Parent/Legal Guardian Authorization for Minor to Access Own MyChart Account form (see “Full Access Patients 14-17 Years”)
Full Access (Patients 14-17 Years)
Provides parents and legal guardians full access to all MyChart features through their own MyChart log in. Full proxy access requires authorization by both the patient (teen) and the parent/legal guardian.
NOTE: Access will not be granted until both the parent request and the teen consent are submitted and approved.
To request full proxy access to a teen’s MyChart information, a parent or legal guardian must review, sign, and submit the MyChart Proxy Request – Full Teen Access form. The teen patient must have their own unique email address. Once submitted, the teen patient will need to review, sign, and submit the MyChart Teen Consent Form – (Teen Authorizing Adult’s Full Access).
Full Access (Patients 18+ Years)
Authorize another person to access your MyChart information to help with your care. Proxy access can be stopped at any time by the proxy or the patient.
To request another adult to have proxy access to your MyChart information, sign into your MyChart account and send an invitation or review, sign, and submit the MyChart Proxy Request Form.
